How to use it

  1. Fill in the medication list first, copied exactly from the labels. The checklist only records whether things were given as that list says.
  2. Print one checklist per week and keep it with the medications or on the fridge, where every helper can reach it.
  3. Whoever helps ticks what they did and initials the "Who helped" column.
  4. Write anything unusual in "Notes for the doctor": a missed dose, a change in appetite, a fall, a question.
  5. Keep recent weeks in the binder and bring them to appointments.

The checklist is a record, not medical advice. It doesn't tell anyone what to give or when. If a dose was missed or something seems off, write it down and ask the prescriber or pharmacist; don't adjust anything on your own. In an emergency, call 911.
